| Summary | In spite of the improvement of one-year graft survival after kidney transplantation 39% of the transplants are lost due to chronic deterioration of graft function. Only death with a functioning graft – most often observed in association with cardiovascular events or malignancy – is a more frequent cause of graft loss. Calcineurin inhibitors contribute due to their side effect profile to chronic damage of the kidney graft and to the development of cardiovascular events and malignancy after transplantation. Therefore, replacement of the calcineurin inhibitor with a non-nephrotoxic drug that has also been associated with a lower incidence of post-transplant malignancy might be an alternative under certain circumstances. Conversion from a calcineurin inhibitor to an mTOR inhibitor is problematic in case of advanced chronic allograft dysfunction. A rapid conversion without long overlap of the calcineurin inhibitor and the mTOR inhibitor treatment should be performed. During the last years the early replacement of calcineurin inhibitors with an mTOR inhibitor as a measure to prevent deterioration of graft function has been shown to be more succesful than the late conversion that is performed as a rescue therapy once a significant deterioration of graft function has already occurred. |
